QUALIFICATIONS:
• Successful completion of an approved RN or RPN program
• Current registration with the College of Nurses of Ontario
• Evidence of strong clinical skill sets
• Previous experience with electronic medical records (EMR) an asset
• Demonstrated ability to work both independently and effectively in a multi-disciplinary
team environment
• Excellent communication, organizational, multi-tasking and problem-solving skills.
DUTIES:
• Assessment of patient health care needs and treatment outcomes. Recording of vitals,
patient history, biometric measurement, etc.
• Coordination of diagnostic, therapeutic and management plans with the patient and
family doctors
• Procedures including immunizations, injections, immunotherapy, venipuncture, ear
syringing, dressings, assisting in minor surgery, urinalysis, pregnancy testing
• Participate in the development and implementation of all of the PSFHT’s general and
targeted clinical programs, as required
• Help patients to identify and use health resources, involve patients in decisions about
their own health, and encourage patients to take action for their own health
• Initiate and support health education activities
• Liaison with community agencies, hospitals, diagnostic and therapeutic services,
pharmacies
• Collaboration with other team members to optimize patient health
• Provide communication to hospital and community staff and other interdisciplinary team
members to ensure smooth transition for care
• Referrals to community services, as appropriate
• Other duties as assigned
